PAPER REGISTRATION and IMPORTANT FORMS

• MEDICAL TREATMENT RELEASE FORM-IMPORTANT: The Archdiocese of Detroit requires that we have a medical consent form on file for each student. Once you submit the online registration form, please fill out one medical form for each child, save the PDF, and email it directly to [email protected].

Please register for all Faith Formation Programs by August 31.
Registrations received after August 31st, will be assessed a late fee of $50.00. Late registrations will not be processed until after the first week of classes and students will be placed where there is availability. Any special needs or requests (classmates, preferred catechist, allergies, etc.) must be included on the registration form in the designated area. After September 8th any changes will be processed after the first week of classes.

ATTENDANCE POLICY: For Kindergarten to Grade 8 – If a student misses more than five class sessions they will not be considered as having completed that grade. Students are responsible for missed material. 

SACRAMENT PREPARATION:  Generally, the Sacraments of Reconciliation and First Communion are celebrated in the 2nd grade, and the Sacrament of Confirmation is celebrated in the 8th grade.  To receive a sacrament this year, your child needs to have participated in Faith Formation last year. If your child was not registered in our program last year, please provide a letter confirming participation from the parish or school where Faith Formation took place.  If you have any questions please call the Faith Formation office at 248-647-4680.

SAFE ENVIRONMENT: Each parish must follow the guidelines and policies established by the Archdiocese of Detroit - Criminal background checks, Protecting God's Children. For ALL Adult volunteers, Called to Serve For ALL Teens. Empowering God's Children For ALL Students.

PAYMENT POLICY:  Payment is expected at the time of registration.  Education tuition will not exceed $420.00 for any family registered in the parish (this does not include out-of-parish or late registration fees). Please note: you must be a registered parishioner for at least 6 months to have the out of parish fee waived. It is our policy that every child is entitled to attend our programs regardless of their ability to pay. If you are experiencing financial difficulty, please contact the Religious Education Office to discuss your options. Payment by credit card can be made online. Catechists receive a 50% discount on their children's tuition.

WEATHER, UNSCHEDULED EVENTS, AND CHANGE OF SCHEDULE:  In case of an unscheduled special event, change of schedule, or cancellation of a class, notification will be sent through myParish App.  Please text App to 88202 to download the app. Select St. Thomas More after opening the app and join the Faith Formation Group. Normal text messaging rates may apply to messages you receive.

PHOTOGRAPH/VIDEO PERMISSION: Please be advised pictures of your child(ren) may be taken and used on our website, bulletin, Facebook page, and/or other forms of media. Please submit a signed and dated letter by August 31 of this school year to the Faith Formation Office stating you DO NOT want your child(ren) to be photographed it must be signed and dated.
